If you choose to see an out-of-network provider—may mean higher out-of-pocket costs. To get reimbursed, submit a claim and itemized receipts for partial reimbursement.
When using your benefits overseas, you'll need to file an out-of-network claim. Visit any international eye care provider and you’ll be reimbursed 75% of billed charges up to the amounts shown.
